چکیده

• Objective: To compare rates of revision knee replacement (KR) surgery for patients who had 2 knees replaced simultaneously with rates for those who had staged surgery. • Design: Retrospective database study covering the period January 1985 through December 1990. • Participants: 11,771 Medicare recipients in the United States. • Main outcome measure: Rates of revision following primary bilateral KR surgery. • Results: Revision rates were lower for patients who underwent simultaneous surgery relative to patients who underwent staged surgery. After adjusting for risk factors previously determined to affect likelihood of revision, patients who underwent simultaneous KR had an approximately 20% lower risk of subsequent revision surgery compared with patients who had staged surgery (risk ratio, 0.79; 95% confidence interval, 0.64–0.98). • Conclusion: The lower revision rates associated with simultaneous bilateral KR is a factor that physicians and patients can consider when making treatment decisions for bilateral disease.

Simultaneous or Staged? A Comparison of Bilateral Uncemented Total Knee Arthroplasty

We analysed the results of both simultaneous and staged total knee replacements (TKR) and compared them to unilateral TKR. The bilateral procedures have significantly higher rates of complications than unilateral cases, almost entirely due to thrombo-embolic problems. This does not however correspond to an increase in mortality rate. If a bilateral procedure is indicated then a simultaneous pro...

Outcomes of unilateral and bilateral total knee arthroplasty in 238,373 patients

Background and purpose - There is no consensus about the outcome of simultaneous vs. staged bilateral total knee arthroplasty (TKA). We examined this issue by analyzing 238,373 patients. Patients and methods - Demographic, clinical, and outcome data were evaluated for TKA patients (unilateral: 206,771; simultaneous bilateral: 6,349; staged bilateral: 25,253) from the Canadian Hospital Morbidity...

A comparison of bilateral uncemented total knee arthroplasty: simultaneous or staged?

In a series of 1304 patients (1867 knees), the results of simultaneous and staged bilateral total knee arthroplasty were compared with each other and with unilateral total knee arthroplasty. The bilateral procedures had a significantly higher rate of complications than unilateral procedures, almost entirely because of thromboembolic problems. However, this did not correspond to an increase in m...

Staggered bilateral knee arthroplasty: good or bad?

To the Editor: I read with interest the article by Memtsoudis et al. in the recent issue of ANESTHESIOLOGY on perioperative outcomes after unilateral and bilateral total knee arthroplasty. When the authors discuss about the issue of benefits and risks of simultaneous versus staged bilateral total knee arthroplasty, they have quoted an article by Sliva et al. In the original article by Sliva et ...

Costs of staged versus simultaneous bilateral total knee arthroplasty: a population-based study of the Taiwanese National Health Insurance Database

BACKGROUND Bilateral total knee arthroplasty (TKA) is required for many patients. There are few studies comparing the overall costs of staged and simultaneous bilateral TKA. METHODS The Taiwan National Health Insurance Research Database (NHIRD) was searched, and the total medical costs of 452 patients who received simultaneous bilateral TKAs were compared with those of 690 who received staged...
